- Out of practice boundary area We can offer registration for patients who live outside of our catchment area but only if it clinically safe to do so. This is usually where the patients works closer to the surgery than a home address and accessing clinical care can be problematic. Each application is reviewed by a clinician to ensure that the clinical care of the patients can be safely provided given the distance of the surgery, and community services they may require, from the patient’s home address
How to Register
To register with our practice, please follow the link below to complete the online registration form.
After you have completed and submitted the form you must attend the surgery within 7 days with two forms of identification.
Acceptable identification includes:
- Birth Certificate
- HC2 Certificate
- Rough sleepers’ identity badge
- Hostel Registration/mail forwarding letter.
Until we have seen your identification you will not be registered at the practice.
